Leen View Surgery is a dementia-friendly. All our staff have taken part in General Practice specific dementia awareness sessions.
We welcome any feedback about how we are doing from patients, families and carers.

Are you a Carer?
If you care for somebody and need extra support please let us know, we can give you details of the help that is available and with your consent refer you to the Carer’s Hub. Visit www.carerstrustem.org for more information or call 0115 8248824.
We are also in the process of developing a Carer’s Pack which will contain lots of useful information and contacts. If you do care for somebody please let us know and we will also add you to our carer’s register. This is so we can identify who may need help & support, it also entitles you to a free NHS Flu Jab. Please ask at reception for more details.
